Decisions: what was actually decided, one line each.
Actions: who does what by when. Use the name as spoken in the transcript. If no owner is named, write "unassigned" rather than guessing.
Open questions: what was raised and left unresolved.
Leave a section out entirely if the transcript contains nothing for it. Do not summarise the discussion itself.

Notes from the author
The instruction to drop empty sections matters more than it looks - otherwise you get an "Open questions" heading with a fabricated question under it. Long transcripts are fine on Gemini and Claude in one call; on smaller context models split by agenda item.
